Immigrant Visa Forms

Depending on the type of petition/application being filed, one of the following forms will be required.

For immediate relative and family based immigrant petitions except for widow(ers):

• I-130, Petition for Alien Relative
• Instructions for Form I-130

For widow(ers) and special immigrant’s petitions:

• I-360, Petition for Amerasian, Widow(er), or Special Immigrant
• Instructions for Form I-360

For fiancé(e) and K3 petitions:

• I-129F, Petition for Alien Fiancé(e)
• Instructions for Form I-129F

For all employment based petitions:

• Form I-140, Immigrant Petition for Alien Worker
• Instructions for Form I-140

For entrepreneur petitions:

• Form I-526, Immigrant Petition by Alien Entrepreneur
• Instructions for Form I-526

For all immigrant visa applicants:

• Form DS-230, Application for Immigrant Visa and Alien Registration

For K visa applicants:

• Form DS-156, Non-Immigrant Visa Application
• Form DS-156K, Nonimmigrant Fiancé(e) Visa Application

For returning resident applicants:

• Form DS-117, Application to Determine Returning Resident Status

For sponsors of immigrant applicants in family based categories:

• Form I-864, Affidavit of Support
• Form I-864A, Contract between Sponsor and Household Member

For sponsors of K visa applicants, certain employment based applicants, and returning residents:

• Form I-134, Affidavit of Support

For minor children of U.S. citizens (biological and adopted) who are admitted into the United States while under 18, and widowers:

• Form I-864W, Intending Immigrant's Affidavit of Support Exemption
